Referral Information for Case Managers
Thank you for thinking of Freedom Adult Care. This page has what you need to send us a referral. If anything is missing, call the office and we’ll get it to you.
What We Provide

Through our Family Hire program, we can hire a member’s family member as their paid PSS aide, so the member gets help from someone they know and the family member can keep other work.
We’re selective about who we hire. When a PSS referral doesn’t have a family caregiver, we’ll tell you honestly how soon we can staff it.

Personal Support Services: Chattahoochee, Harris, Marion, Meriwether, Muscogee, Schley, Sumter, Talbot, Taylor, and Troup.

How to Send a Referral

An EDWP or referral form by email is fine. You don’t need to call first.
Provider Numbers
Need our Medicaid provider numbers for a service authorization? Call or email the office and we’ll send them the same day.
